Girls Lacrosse Driver's Form
All drivers need to fill out the Use of a Private Vehicle for Transporting Students Form in order to drive any student, other than your own, to a sport event. You must also provide a copy of your insurance showing appropriate and effective coverage. A fillable pdf form can be found at Driver Form . Just print and bring into the school office with the correct paperwork. If you have filled this form out for another 2012-2013 Sport during Fall or Winter Season, you should be covered for the year.
